• Identifying and initiating referrals to crucial social service programs, including financial, psychosocial, and state supportive services.
• Managing the member’s care plan throughout the entire continuum of care, ensuring quality and coordinated services are provided.
• Travelling up to 75% of the time within your dedicated territory to visit members in their homes and other community settings.

The Profile of a UHG Clinical Professional. We are looking for a seasoned clinical professional with strong community health experience. The required qualifications include:

• A current, unrestricted RN license in the state of Texas.
• 2+ years of experience working within a community health setting or in a similar health care role.
• A valid driver’s license, reliable transportation, and the ability to travel up to 75% of the time.
• Proficiency with Microsoft Office (Word, Excel, Outlook).
• A dedicated home office with high-speed internet.
